Frequently Asked Questions

As a healthcare partner, when it is my responsibility to call Infinite Legacy?

Healthcare partners should call Infinite Legacy at 800-923-1133 after a patient’s death has been declared by the hospital doctor(s) or if a patient’s family asks about organ donation. Infinite Legay will determine if the patient meets the criteria to be an organ, eye or tissue donor.

If a patient is on a ventilator, call Infinite Legacy within two hours if any trigger is met to screen for organ donation potential:

  • Before meeting to discuss end-of-life options or de-escalation of life-sustaining therapies (palliative care/hospice consults, DNR for purposes to not escalate care, etc.)
  • GCS<5

If Infinite Legacy is monitoring a patient, call us back if:

  • Brain death testing is planned
  • End-of-life conversations are planned
  • The patient becomes unstable

Please call Infinite Legacy within 60 minutes of a cardiac death for ALL patients to screen for tissue donation.

HIPAA’s privacy rule is, “a covered entity (hospitals) may use or disclose protected health information to organ procurement organizations (OPOs) or other entities engaged in the procurement, banking or transplantation of organs, eyes or tissue for the purpose of facilitating organ, eye or tissue donation.” This rule allows Infinite Legacy and other OPOs to access protected health information for donors and potential donors without authorization. This helps us determine medical suitability and facilitate deceased organ donation.
Patients who are on a ventilator with a GCS of 5 or less for whom brain death testing is planned, can have the potential to be an organ donor. Additionally, any patient who may qualify for terminal extubation, DNR status or “allowing natural death” can be considered for organ donation.

Any patient who has been declared dead by a hospital doctor(s) may be eligible to donate tissue. Patients who cannot qualify to donate organs may still be able to donate their corneas and other tissues. All deaths must be called in to Infinite Legacy within 60 minutes of death declaration.

Infinite Legacy is the federally designated organ procurement organization (OPO) for Maryland, Northern Virginia and Washington, DC. We recover organs, eye and tissues and provide lifesaving transplants to those on the national transplant waiting list.

Calling Infinite Legacy will give our staff more time to determine a patient’s medical suitability, donor designation status and develop a plan for discussing donations with the patient’s family is approved. This is the first step in the donation process. Calling in a referral is required by the Centers for Medicare & Medicaid Services (CMS), The Joint Commission and your hospital policy.

Anyone on the healthcare teams that have access to a patient’s medical information in our service area in Maryland, Northern Virginia and Washington, DC can call Infinite Legacy.

The hospital care team and Infinite Legacy staff are two separate teams who work together during donations cases. The hospital care team are experts in their respective fields and do everything they can to save a patient’s life. If death is declared by the hospital, Infinite Legacy staff are trained specifically in caring for families through end-of-life discussions and donation decisions. When organ, eye or tissue donation is an option, the hospital team and Infinite Legacy staff work together to support the family’s decision.
Hospital staff can tell families that donation may be an option for their loved one and that they will contact Infinite Legacy to facilitate a conversation about potential organ, eye and tissue donations. The hospital staff will then contact Infinite Legacy and one of our team members will reach out to the family about the donation process. Infinite Legacy will then explain all the options that are available to the family.
If any patient meets the referral triggers, please contact Infinite Legacy at 800-923-1133. If any patient’s plan of care changes, code status changes or shows signs of instability, call Infinite Legacy and communicate all updates. The hospital care team must also maintain the patient’s hemodynamic status for Infinite Legacy to assess eligibility for organ, eye and tissue donation and speak to the patient’s family about available donation options. The hospital care team and Infinite Legacy must maintain good communication to ensure donation remains an option while the family considers end-of-life decisions.
