Authorization to Share

Thank you for uploading a Stories of Hope submission. The Infinite Legacy Communications and Marketing Team will follow up with you via email to request you sign the Infinite Legacy Publicity Consent and Privacy Policy Release Form authorizing us to share this story either on your behalf or as LNOK on behalf of your loved one.

In allowing Infinite Legacy to facilitate the Stories of Hope, I hereby waive any rights to inspect and/or approve the finished product. I am aware that I will not inspect and/or approve the finished product. I am aware that I will not receive any compensation for my participation and/or development of this story. I further understand that the materials may be used in the future without my further authorization. I understand that I may revoke authorization by providing a written statement to Infinite Legacy, 1730 Twin Springs Road, Suite 200, Baltimore, MD 21227. After that written revocation is received and processed, no further information shall be shared by Infinite Legacy.
